Installed it for a Sugar CE 5.2 Installation and it worked like Charm out of the box. Re the Fields missing - just create them in Sugar - not more than 3 min of effort.

I use to work and love this extension, but since the Sugar CRM guys have removed the portal-associated fields in the contact module (except for the entreprise edition), this module is now obsolete. The latest posts late 2008/early 2009 on the developer web site make me think that this extension is not further maintained.

Indeed, I haven't been actively developing this plugin for some time now, exactly for the reasons you've given. The folks at SugarCRM Inc seem less then motivated on providing a sound API. There are workarounds to bypass the portal-associated fields, however. I'll be happy to give you some pointers in the right direction. And in the broader sense: you are free to contribute to development of this plugin off course.
Very easy to set up. Once everything is in place you can have your contacts in your SugarCRM database log in with their email address, first name, last name or any user name you want to give them. Very convenient! The plugin then creates a Joomla user account for that contact.
I had problems first but that was related to Sugar 5.0 and the author was more than helpful and responded with a bux fix.
