Forms CRM 3.0 and 2.5 are based on the popular Forms LT CRM 1.5. It allows users to create custom forms with custom fields and embed in any article via a mosbot plugin. User can create multiple forms for different purposes including custom user registration, reservation forms, event forms, submit articles, support question forms, pre-sale forms, etc. The forms also allow for Google conversion tracking code, as well as others. Ideal for Joomla 2.5 and 3.0 sites focused on LEAD GENERATION.

Once the user data is captured, the lead can be assigned to STAFF member automatically. The Staff member can communicate through the front end CRM directly with the customer, who also can log in and leave comments, attach items, etc.

System also includes a MODULE for FORMS, and Tell A Friend Module. Tell a friend data is also stored in the CRM.

Backend allows admin to review leads, read comments between customer and staff, and add PRIVATE comments for STAFF.

Admin can also EXPORT data as normal CSV, or set custom field and text deliminators.

Looked at a couple of contact forms extensions but settled on this, and glad I did.
Relatively easy to use compared to some, and the tutorials on the dev's website are very helpful. There's no "forms page" as such, just create your form in the admin side and then embed a form in an article via the link on the article's editing page. Want a "forms page"? Make one :-)
It's simple if you want it to be and there's a full CRM there with contact logging in admin.
Had an issue installing, which was entirely my own doing because I didn't read the tutorials first. There's four packages in the download, install them separately.
I've had cause to contact the dev once and my query was answer in minutes - excellent!
