Really full customizable. You can use 2 dropdown boxes and telephone, and you can customize the labels and selections of them.
It use's the default Joomla sending mail system.
You can use it for free under GPL.

In regards to the Text box being small. Open 'mod_fastcontact.php' and near the bottom, you will find:

Very quick and easy to set up, and no apparent bugs.
If you want a basic contact form on you site, then this will do it. This module does away with the complications/confusion on setting up a simple form. It can be set in a module position or article. No captcha with this, but I did want it.

The software module is non-functional. The upload was successful but enabling the module did not display fast contact. The effort to debug has been a complete waste of time. Reading their website, I also tried embedding fast contact in an article per instructions. This did not work as well.
Joomlabyte offers extremely slow respond to email (if at all). My experience is they do not respond unless one buys the 'commercial' version, which incidentally is non-functional as well.

Also feel free to ask or report bugs on our components, modules and templates
as other said easy to install and configure, but .... they hide the class file (css) in their website not with installation package and force you to show up their Name with a link to their website or to donate if you want to hide it !!! and if something happened to their site ... goodbye as the module will not work.

Very easy to install and set up. I spent five minutes configuring and the first test worked seamlessly. Really nice for a site that needs a quick and easy contact form. - warning there is no captcha feature - but I did not need this. I needed quick/easy and working. This worked straight out of the install.

Download was easy but required registration. Installation was a snap and adding it into the article was quick once I figured out how to do that (see this how to if you're stuck: http://docs.joomla.org/How_do_you_put_a_module_inside_an_article%3F).
The only thing I wish I could change is the size of the message box. It's a bit small. But, considering the functionality and price, this module is fabulous!
