This was a breeze to install, but took a bit of time to get used to for setting up my first few forms. Had a couple of issues with GPG, but with stellar support was able to get it solved.
These guys are heroes when it comes to support, and they are very fast!
I give them the top rating for my experience with them.

Not being a programmer, I rely on well built tools to create Joomla sites for my clients. Phil Taylor builds great tools and Joomla Forms is a perfect example.
I had a late night email from a client "requesting" a form to be added to their site for a presentation to their boss the next day. I'd just received an email from Phil with a link to demo Joomla Forms.
I've used his previous form tool - Phil-A-Form (Joomla 1.0) - with success so I checked out the demo. Half an hour later I had downloaded, installed and used Joomla Forms to create a perfect form on my client's site.
Joomla Forms has enough options to cover most of the typical requirements for an online form running in Joomla. The documentation on how to use those options is solid and a sample form is included so you can see exactly how it works. The online forum is well organized and Phil is active in answering questions and solving issues.
Even if I were a programmer, I'd use this tool because it's fast, flexible and reliable.

This is hands down the easiest and most powerful form creator available for Joomla 1.5. I was sold after using the demo for 5 minutes. There are a couple of other very good form generators, but this one blows them both out of the water. Try the demo out and see for yourself.
Having tried most of the Joomla 1.0 and a few of the Joomla 1.5 Form Generators, I can say without doubt that this is an extremely intuitive extension.
Within an hour I had it installed and working with a unique form of my composition.
