This module displays a dropdown menu with Proforms' forms.
The dropdown menu can include all available forms or the forms of a special category.
When selecting a form name from the list the site will be redirected to the selected form page.

This module is non commercial and can be downloaded
I recommend this form to all of them who need something like a this. I mean an extensive form, like university applications, concurs applications, and so on. The form work great in multilanguage site. Does not affect at all any plugin and your content in your site. It works great with edocman integrated too.
