Everything works amazingly with this component. It's extremely easy to figure out and it is kind of documented well. The only problem I have had is that when setting up multiple check boxes, none of the data is sent to my e-mail.

This was a little bug in 1.1.9.
You can read F.A.Q.'s. There is an instruction to resolve this problem.
Upcoming small update 1.1.9.1 will purge a several little bugs, also this one.
