BreezingForms
This is one of the best Form extension. I especially like the plugin which allows me to use forms in my articles. BreezingForms has many functions and it may be a bit hard to use at first or for a bit more advanced forms but you can probably get support at the forums if you have any problems. I still haven't figured out all I need for my forms, but I'm getting used to it. The price is very acceptable for this extension!
