This module is not fully J2.5 compatible as indicated. While the module worked in previous versions it does not work as described. Until it is fixed I would suggest searching for another solution.

The component and plug-in are easy to use and add custom scripting to handle additional validation. The support from Bogdan was very helpful and responses were very timely. If you're looking for a way to quickly build forms and do custom scripting, here is the component to use.
