BF Survey Basic
I was looking for a standalone Survey/Feedback solution for a client and after trying most of the ones available I decided on BF Survey Pro.
Although it is not designed to be a standalone product with the fantastic support from it's developer Tim I was able to make it just that and now I have a very nicely packaged product AND a very very happy client as a result.

Some drawbacks are that in order to achieve this several hacks needed to be made to core Joomla files, however this was minimal and easy to keep track of should the need arise to upgrade either Joomla or BF Survey Pro component.

The component is very feature rich and easy to use with very few limitations. It's basically everything you could possibly ask for from a survey component including a graphical statistics overview of responses in both admin AND front!!

Highly recommended component and well worth the pennies!!
