This plugin is very handy but I had problems when SEF URLs is enabled. The fix I worked out is a small modification in the code on line 80 of plugins/content/comascon.php:
Change src='index2.php?' to src='/index2.php'

This is a great tool for generating forms. There are a lot of features here and it works great. Being able to send the form contents based on what is selected in the form is most valuable.
