This form extension has a lot that can be done with it. And if you need to extend = it even further, there are a number of ways that you can add your own js or php code to customize your form.

I'm with Yoda below. I just installed the feature rich Sexy Lightbox. Their products are 5 stars/support even more.

Everything worked great, but I struggled to remove the scroll bars from the pop-up lightbox. My template css was conflicting with the lightbox css. I left a forum post on a Saturday, thinking I would get an answer later next week.

Within 24 hours - on a Sunday no less - they answered my post with a simple rule to add to my template css to remove the conflict. Outstanding support.

Thanks for the product/support. Support is very important to me and I'll be looking over your other products because of it.
