Works OK but a few glitches

Does what it says, but tweaking is not straightforward or obvious. I was unable to get the address display to show typical USA city, state despite configuring it as such. Modifying the page via the language ini file had no effect on rendering. Recaptcha display broke my page display, but Check This Box set to mandatory seems to be a workaround to confirm human.

I submitted a request for guidance on how to solve my concerns, will see if support is forthcoming.
