This extension is probably ideal for you unless you are doing HTML5 and not Flash. In HTML5 mode you need to be able to define multiple video sources as you need at least 2 formats to cover major browsers (H.264 and Ogg Theora). Current options allow for just a low-res and a hi-res option.

If you're looking for a basic contact module this one is good.

If you are developing templates then unfortunately this module doesn't support template overrides as it doesn't follow the MVC pattern.
